1. This Regulation determines the form, volume, procedure and conditions for the provision of social support, carried out through reimbursement of expenses.

2. Regulation on the reimbursement of expenses for providing children of the first - second year of life with special dairy products baby food, as well as information about healthcare organizations providing services to provide appropriate products, is posted on the Internet information and telecommunication network on the Unified Portal of State and Municipal Services "Public Services" (www. *****), on the Internet site "State and municipal services of the Tyumen region" (www. uslugi. *****).

3. Reimbursement of expenses for providing children of the first - second year of life with special dairy products for children's nutrition is carried out in relation to families living in the Tyumen region with an average per capita income not exceeding 5,000 rubles.

4. Expenses for providing children of the first - second year of life with special dairy products for children's nutrition are reimbursed to healthcare organizations that provide services for the provision of appropriate products (including through dairy kitchens, pharmacy organizations) within the framework of the territorial program of state guarantees for the provision of free medical care to citizens.

5. Provision of children of the first - second year of life with special dairy products for children's nutrition as a measure of social support and reimbursement of the corresponding expenses are carried out within the recommended list of special dairy products for children's nutrition in accordance with Appendix No. 1 to the Regulation. The volume and type of special dairy products for children's nutrition for a particular child is determined by the doctor (paramedic) who writes the prescription, depending on age, health status and individual features child development.

6. Special dairy products for children's nutrition are provided by the organizations specified in paragraph 4 of the Regulations to a family with children (children) of the first or second year of life, free of charge on the basis of a prescription from a doctor at a children's clinic at the family's place of residence.

7. The basis for issuing a corresponding prescription by a doctor (paramedic) is information on families entitled to reimbursement of expenses for providing children of the first and second years of life with special dairy products for children's nutrition, formed by the Department of Social Development of the Tyumen Region and provided to the health care organizations of the Tyumen Region by the Territorial Fund compulsory medical insurance in the Tyumen region.

8. In the absence of data on the relevant family in the information provided by the Territorial Compulsory Medical Insurance Fund of the Tyumen Region to the healthcare organization, one of the child's parents or a person replacing him (adoptive parent, guardian, adoptive parent), for the doctor (paramedic) to issue an appropriate prescription, provides a certificate from the territorial department of social protection of the population on the possibility, based on paragraph 3 of the Regulation, of free provision of special dairy products for children's food.

To obtain a certificate, one of the parents or a person replacing him (adoptive parent, guardian, foster parent) submits in person or by mail to the territorial office social protection population (hereinafter referred to as the Department) at the place of residence (place of stay) an application in the form in accordance with Appendix No. 2 to this Regulation.

An application for issuing a certificate for the free provision of special dairy products for baby food can be submitted through a multifunctional center for the provision of state and municipal services (hereinafter referred to as the MFC). b) information about the identity document of the applicant (type of identity document, series, number, by whom, when the document was issued);

c) information about the place of residence, place of stay of the applicant (name of the region, district, city, other settlement, street, house number, building, apartment) is indicated on the basis of an entry in the passport or a document confirming registration at the place of residence, place of stay (if not a passport is provided, but another identity document);

d) information about the place of actual residence (postal code, name of the region, district, city, other settlement, street, house, building, apartment number, contact phone number);

e) information about the composition of the applicant's family and the income received by members of his family for the last three calendar months prior to the month in which the application was submitted;

f) the postal address (e-mail address) of the applicant, to which notification of the decision must be sent.

9. The following must be attached to the application:

a) a copy of the passport or other identity document of the applicant;

b) a certificate (extract from the apartment card, house (apartment) book) about the applicant's family members registered in the residential premises, if the housing does not belong to the municipal housing stock. The certificate is issued by an organization that calculates payments for housing at the place of residence;

c) documents confirming income for the last three calendar months preceding the month of application (with the exception of income received in the form of a pension in the territorial departments of the Branch pension fund RF in the Tyumen region, measures of social protection of the population of the Tyumen region, received in the departments of social protection of the population of the Tyumen region, unemployment benefits, received in the territorial employment centers of the Tyumen region).

At the request of a citizen, the following can be attached to the application:

a) a certificate (extract from the apartment card, house (apartment) book) about the applicant's family members registered in the residential premises, if the housing belongs to the municipal housing stock;

b) a copy of the birth certificate of the child (children);

c) documents on income received in the form of a pension in the territorial departments of the Branch of the Pension Fund of the Russian Federation in the Tyumen region, measures of social protection of the population of the Tyumen region received in the departments of social protection of the population of the Tyumen region, unemployment benefits received in the territorial employment centers of the Tyumen region for the last three calendar months preceding the month in which the application was submitted.

The processing of the information contained in the application and the documents specified in this paragraph is carried out using the information system of the social protection authorities of the Tyumen region.

10. If an application is submitted with all the documents specified in paragraph 9 of this Regulation attached, the specialist of the Department registers the application in the appropriate registration log and issues a receipt to the citizen on acceptance of the application, then considers the application and the documents attached to it on the day the application is submitted. After reviewing the application and the submitted documents, the specialist of the Department prepares a certificate in the form in accordance with Appendix No. 3 to this Regulation and, at the request of the citizen, hands it over to the citizen or sends it to the postal address indicated by him. If there are grounds for refusing to issue a certificate, a written notice of refusal indicating the reason for the refusal, at the request of the citizen, is handed over to him or sent to the postal address indicated by him on the day of filing the application.

11. On the day of receipt of the application sent by mail, the specialist of the Office registers it in the appropriate registration log and within 1 working day from the date of its registration sends to the address indicated in the application a notification of receipt of the application indicating the list of accepted documents, the date of their receipt and assigned registration number. In this case, the preparation of the certificate is carried out within 2 working days from the date of receipt of the application by the Department with the attachment of all the documents specified in paragraph 9 of these Regulations.

Within 3 working days from the date of registration of an application submitted without attaching documents provided at will, a specialist of the Department requests these documents from the relevant authorities, organizations and institutions, including through the automated system of interdepartmental electronic interaction of the Tyumen region (SMEV).

The preparation of the certificate in this case is carried out within 3 working days from the date of receipt by the Department of confirming information from the relevant authorities, organizations and institutions.

12. Accounting for income and calculation of the average per capita income of a family for the free provision of children of the first and second years of life with special dairy products for baby food is carried out in the manner established for accounting for income and calculating the average per capita income in order to recognize citizens as poor and provide them with state social assistance.

13. The grounds for refusal to issue a certificate are:

a) non-compliance of the applicant with the requirements established by paragraph 3 of these Regulations;

b) identification of incomplete and (or) inaccurate (distorted and inaccurate) information in the submitted documents.

14. The specialist of the Department notifies the applicant about the issuance of the certificate and the possibility of obtaining it within 3 working days from the date of its issuance by phone or at the e-mail address indicated in the application, at the request of the applicant sends the certificate to the postal address indicated in the application.

A written notice of refusal to issue a certificate indicating the reason for the refusal is sent to the citizen at the postal address (e-mail address) indicated in the application within 3 working days from the date of the relevant decision.

The issuance of a certificate is the basis for entering relevant family data into the information system of the social protection authorities of the Tyumen region.

15. The Department of Social Development of the Tyumen Region, on a monthly basis, before the 5th day of the month following the reporting one, provides data on families eligible for reimbursement of expenses for providing children of the first and second years of life with special dairy products via electronic communication channels to the Territorial Compulsory Medical Insurance Fund of the Tyumen Region baby food.

The Territorial Compulsory Medical Insurance Fund of the Tyumen Region, on a monthly basis, before the 10th day of the month following the reporting one, provides the above data via electronic communication channels to the health care organization of the Tyumen Region.

16. Reimbursement of expenses for providing children of the first - second year of life with special dairy products for baby food is carried out by the organizations specified in paragraph 4 of the Regulations at the expense of the territorial fund of compulsory medical insurance of the Tyumen region in the amount and manner established in accordance with the agreement concluded between the fund and the relevant organization.

17. The organizations specified in paragraph 4 of the Regulations, monthly, by the 5th day of the month, submit to the Department of Health of the Tyumen Region a report for the past month in the form approved by the order of the Department of Health of the Tyumen Region.

Pediatricians, gynecologists and other doctors decide what kind of food children and women will receive free of charge within the framework of state assistance. The diet they choose should help healthy eating, full development child's body.

According to the all-Russian trends, in 2020 the list of products issued in the so-called dairy kitchen includes:

  • puree from vegetables and meat;
  • pasteurized cow milk;
  • adapted for the child's body kefir.

The list is supplemented by nutritious and medicinal mixtures, liquid or dry, purees made from fruits, milk curds, juices made from fruits.

It is impossible to get anything from this list of products without having an official prescription from a doctor.

The amount of food a baby will receive for free depends on its age. The composition of the food basket also depends on age.

  • For example, in Moscow, a child who is six months old has the right to receive a dry mixture once every 30 days - 350 g and juice made from fruits - a little more than 1 liter. Expiry date of a dairy recipe


It is generally accepted that the prescription is issued for a period of 1 month. In rare cases, this period is extended by 3 months. For example, if the child is not yet three years old, if the recipient of the food is a pregnant woman or a nursing mother.

A six-month prescription is available for disabled children and children from large families.

(DAIRY CUISINES) ON THE CONCLUSION OF DOCTORS IN THE URBAN DISTRICT

CITY OF NIZHNY NOVGOROD


1. This Regulation establishes the procedure for providing adequate nutrition for children under the age of three through special food outlets (dairy kitchens) at the conclusion of doctors in the urban district of the city of Nizhny Novgorod.

2. Special dairy products are provided to children under the age of three through special food outlets (dairy kitchens) by prescription. medical workers when transferring a child to mixed or artificial feeding at the place of residence of the parents (legal representatives) with whom the child lives.
A prescription for receiving dairy products for children under the age of two is issued by a medical institution to a parent (legal representative) on the basis of a certificate of the average per capita income of the family issued by the state government institutions of the Nizhny Novgorod Region "Department of Social Protection of the Population", government agencies Nizhny Novgorod Region "Center for the Provision of Social Support Measures", state institutions of the Nizhny Novgorod Region "Comprehensive Center for Social Services for the Population" (hereinafter referred to as social protection authorities) at the place of residence of one of the parents (legal representatives) with whom the child lives.
Special food outlets (dairy kitchens) distribute dairy products for children of the first and second years of life if they have a prescription issued by a medical institution and a certificate of the average per capita family income from recipients of dairy products from the social protection authorities.

3. One of the parents (legal representative) has the right to free food for children of the first and second years of life:
3.1. For children of the first year - with an average per capita income of the family not exceeding the value of two living wages per capita, established by the Government of the Nizhny Novgorod Region on the date of appointment of meals.
3.2. For children aged one to two years - with an average per capita income of the family not exceeding living wage per capita, established by the Government of the Nizhny Novgorod Region on the date of appointment of meals.

4. Children aged two to three years are provided with food at the expense of citizens' personal funds.

5. The calculation of the average per capita income of a family and the issuance of certificates of the average per capita income of the family are carried out by the social protection authorities at the place of residence of one of the parents (legal representatives) with whom a child under the age of two lives, on the basis of:
- statements;
- the applicant's passport;
- certificates (copies of the certificate) of the birth of the child;
- information about the composition of the family (family members registered in this residential area);
- certificates of income of family members for the three months preceding the month of applying for the appointment of free meals (for non-working citizens - copies of the work book, certificates from the employment service, for pensioners - certificates of the amount of the pension received).

6. When a guardian (custodian) applies for the appointment of special dairy products for children of the first and second years of life, a copy of the decision of the local self-government body on the establishment of guardianship (guardianship) over the child is additionally presented.

7. Accounting for income and calculation of the average per capita income of a family are carried out in accordance with the Federal Law of April 5, 2003 N 44-FZ "On the procedure for accounting for income and calculating the average per capita income of a family and the income of a single citizen living alone to recognize them as poor and provide them with state social assistance" and Decree of the Government of the Russian Federation of August 20, 2003 N 512 "On the list of types of income taken into account when calculating the average per capita income of a family and the income of a citizen living alone for the provision of state social assistance."

8. When calculating the average per capita income, the family includes persons related by kinship and (or) property. These include spouses living together and maintaining a joint household, their children and parents, adoptive parents and adopted children, brothers and sisters, stepchildren and stepdaughters.
The fact of running a joint household is indicated by the applicant in the application for the appointment of free meals.

9. Parents (legal representatives) of children of the first and second years of life who are provided with free meals are obliged to report changes in family income to the social protection authorities at the place of residence within a month.

10. If the composition of the family changes, the calculation of the average per capita income of the family is carried out again on the basis of a certificate of family composition and documents on the income of all family members for the three months preceding the application.

11. When applying for the resumption of the provision of free meals, which was suspended due to the excess of the average per capita family income of the subsistence minimum established by the Government of the Nizhny Novgorod Region, the provision of free meals is carried out from the month of application to the social protection authorities and upon issuance of the relevant certificate.

12. Providing free meals to children of the first and second year of life is terminated from the first day of the month following:
- for the period when the average per capita income of the family for three months exceeded the value of two subsistence minimums for children under the age of one year and one subsistence minimum for children aged one to two years per capita, established by the Government of the Nizhny Novgorod Region on the date of appointment of meals ;
- after the month of occurrence of other circumstances affecting the right to receive (moving, transferring the child to full state provision when the child reaches the age of two).

13. Control over the validity and correctness of the issuance of free meals is carried out by the head of the medical institution.

14. Financing the provision of food for children in the first and second years of life is carried out at the expense of the regional budget.

15. Control over the implementation of these Regulations is carried out by the local self-government body of the urban district of the city of Nizhny Novgorod.

1. The right to free provision of special dairy products for children's nutrition is granted to families with children of the first or second year of life, in which the average per capita income does not exceed the subsistence minimum per capita established in the Kaliningrad Region.

2. Bodies of social protection of the population of municipalities according to the documents submitted by one of the parents (adoptive parents, guardians) in accordance with the resolution of the head of the administration (governor) of the Kaliningrad region of January 20, 2000 N 11 "On approval of the list and forms of documents necessary to determine the amount average per capita family income when establishing the right to monthly allowance per child" calculate the average per capita income of families raising children of the first - second year of life, and its compliance with the subsistence level.

3. On the basis of the calculations made, the bodies of social protection of the population form lists of children of the first - second year of life to be provided with free meals, monthly submit these lists to the health authorities of the municipality.

4. Health authorities of municipalities carry out monthly issuance of special dairy products for children according to the lists provided by the social protection authorities, in accordance with the target figures for free distribution of special dairy products for children of the first - second year of life for 1 month and in emergency cases for children of the first - the second year of life, who find themselves without parental care and care, as well as in cases that threaten the health and life of children brought up in families of social risk, regardless of whether they are registered in the social protection authorities of the population of the municipality.

5. Bodies of social protection of the population of municipalities:

Quarterly check the lists of children of the first - second year of life, to be provided with free meals, with the health authorities of the municipality;

Children of the first and second years of life are registered for the provision of free meals at the request of the healthcare authorities of the municipality, taking into account the average per capita income of a family raising children of the first and second years of life, and its compliance with the subsistence minimum.

6. Health authorities of municipalities:

The calculation is carried out for the purchase of the quantity and range of special dairy products for baby food for children of the first - second year of life, who are on a mixed and artificial feeding to be provided with free meals in accordance with the list of children of the first - second year of life, formed by the social protection authorities of the population of the municipality, and the conditions for the free provision of children of the first - second year of life with special dairy products for baby food in accordance with Appendix No. 2;

Based on the calculations made, an order-application for the required assortment and quantity is formed and submitted to the organization that purchases and delivers baby food to the territory of the municipality;

Create a stock of special dairy products for baby food (10% of monthly rate) to provide free meals to children of the first - second year of life who find themselves without parental care and care, as well as in cases that threaten the health and life of children brought up in families of social risk, regardless of registration in the social protection authorities of the population of the municipality;

In order to support breastfeeding the application includes special mixtures for nursing mothers to stimulate lactation at the rate of 1200 grams per 1 month;

Carry out explanatory work in families raising children of the first - second year of life, and among the population on the procedure for providing free meals to children in the first - second year of life with special dairy products for children's nutrition;

Provide a quarterly report to the administration of the municipality and the health department of the regional administration on the use of financial resources and the range of purchased special dairy products.
