AFTER COMPLETING THE INSURED PERSON’S APPLICATION FOR APPOINTMENT

ACCUMULATIVE PART OF LABOR PENSION

1. In the first line of the insured person’s application for the purpose of the funded part labor pension(hereinafter referred to as the application) indicates the name of the non-state pension fund, which the insured person applied to, is entirely in the nominative case.

2. Paragraph 1 of the application indicates information about the insured person who is assigned accumulative part labor pension:

surname, name, patronymic (if any) of the insured person are indicated in full in the nominative case in accordance with the document proving his identity;

in the line “Insurance number of the individual personal account of the insured person” the insurance number of the individual personal account of the insured person is indicated in accordance with the insurance certificate of compulsory pension insurance of the insured person;

in the line “Pension account number of the accumulative part of the labor pension” the number of the pension account of the accumulative part of the labor pension of the insured person is indicated;

in the line “Citizenship:” the word “citizen” or “citizen” is written, depending on who this line is being filled in, and the state of which the person is a citizen (subject) is indicated. If a person has dual citizenship, information about citizenship is indicated separated by commas. information about the residence of the insured person outside the Russian Federation is filled in if the insured person lives outside the territory of the Russian Federation:

In the line “address of residence in the territory of another state” in Russian and foreign (the language of the state of residence) languages, the full address of the place of residence of the insured person in the territory of the state of residence is indicated;

In the line “address of place of residence before leaving the Russian Federation” in Russian, the full address of the place of residence of the insured person on the territory of the Russian Federation before leaving its borders is indicated;

in the line “Name of identification document” the name of the submitted document proving the identity of the insured person (passport, residence permit or other identification document established by the legislation of the Russian Federation) is indicated. 3. Paragraph 2 of the application is filled out in addition to paragraph 1 of the application if an application for assignment of a funded part of a labor pension is submitted by a representative (legal representative of an incapacitated person, an organization entrusted with the duties of a guardian or trustee, a proxy):

in the line “Telephone number” the telephone number of the representative of the insured person (the organization entrusted with the duties of a guardian or trustee) is indicated;

in the line “Name of the representative’s identification document” the representative’s identity document (passport, residence permit or other identification document established by the legislation of the Russian Federation) is indicated; then in the corresponding lines of the application the series, number, date of issue of the identity document are indicated , name of the authority that issued it;

in the line “Name of the document confirming the authority of the representative” the document on the basis of which the representative carries out his functions on behalf and in the interests of the insured person is indicated (certificate of guardianship or trusteeship, power of attorney or other document confirming the authority of the representative to exercise guardianship or trusteeship, established by law Russian Federation), then in the corresponding lines of the application the series, document number (if these details are provided for in the form of the document confirming the authority of the representative), date of issue and name of the authority that issued it are indicated.

4. Clause 3.1 of the application is completed if the insured person has pension savings accounted for in the pension account of the accumulative part of the labor pension, formed from additional insurance contributions to the accumulative part of the labor pension, employer contributions, contributions for co-financing the formation of pension savings, income from their investment, funds (part of the funds) of maternal (family) capital aimed at forming the funded part of the labor pension, income from their investment.

5. 6. In paragraph 5 of the application, depending on whether an urgent pension payment was assigned or not, a mark is made in the appropriate box.

7. Paragraph 6 of the application contains information about which the insured person (his representative) was warned;

in the column “Name of document” the names of the documents attached to the application are indicated. 9. The receipt-notification of the acceptance and registration of the insured person’s application for the assignment of a funded part of the labor pension (hereinafter referred to as the receipt-notification) is filled out by the employee of the non-state pension fund who accepted the application from the insured person (his representative), except for the line “Receipt-notification received )", containing the date, signature of the insured person (his representative), a transcript of the signature, in two copies, one of which is handed over to the insured person (his representative) or sent to him by mail.

10. The first line of the notification receipt indicates the name of the non-state pension fund to which the application was submitted.

11. In paragraph 1 of the notification receipt:

in the line “Application of the insured person for the assignment of a funded part of the labor pension” the surname, first name, patronymic (if any) of the insured person is indicated in full in the nominative case in accordance with the document proving his identity;

in the line “insurance number of the individual personal account of the insured person” the insurance number of the individual personal account of the insured person is indicated in accordance with the insurance certificate of compulsory pension insurance;

in the line “filed in the interests of the insured person by his representative” the surname and initials of the representative of the insured person are indicated;

in the line “and documents submitted with the application” documents accepted simultaneously with the application are indicated:

In the column “Name of document” the names of the documents attached to the application are indicated. The column “Documents returned to the insured person (his representative)” is filled in when documents are returned to the insured person (his representative);

The line “accepted:” is filled in by an employee of a non-state pension fund:

In the column “Registration number of the application”, the employee of the non-state pension fund who accepted the application from the insured person indicates the registration number of the application;

12. Clause 2 of the notification receipt indicates the documents that must be submitted additionally to assign the funded part of the labor pension:

in the line “Documents that must be additionally submitted no later than __________” the date is indicated no later than which additional documents must be submitted to assign the funded part of the labor pension;

in the column "P/n" the serial number of the record about the name of the document is indicated;

in the column “Name of the document” the names of the documents are indicated (certificate of work experience, etc.).

13. In paragraph 3 of the notification receipt in the line “I received the notification receipt”:

in the “Date” column, the insured person (his representative) indicates the date of receipt of the notification receipt;

in the column “Signature of the insured person (his representative)” the insured person (his representative) puts a signature confirming:

Receiving clarification about what documents must be submitted additionally to assign the funded part of the labor pension and within what period;

in the column “Decoding of signature (surname, initials)” the full surname and initials of the insured person or the full surname and initials of the representative of the insured person are indicated.

14. Clause 4 of the notification receipt contains information about which the insured person (his representative) is warned.

15. Clause 5 of the notification receipt is filled out by an employee of a non-state pension fund when the insured person (his representative) submits additional documents after receiving the insured person’s application for the assignment of a funded part of the labor pension in the manner prescribed for filling out paragraph 1 of the notification receipt. In this case, paragraphs 1, 2 and 3 of the notification receipt are not completed.

The line “Last document submitted (date)” is filled in when the additionally submitted document is the last document necessary for assigning the funded part of the labor pension.
